Asian leaf 'kratom' making presence felt in U.S. emergency rooms

When a patient showed up in a West Coast emergency room early this month suffering withdrawal from something he called "kratom," the psychiatrist on duty was forced to scramble for information. But when he looked it up, he found that the opiate-like leaf from Southeast Asia is well known in the worlds of alternative medicine and the drug culture.

What the doctor, who asked not to be named for patient confidentiality reasons, found in an Internet search were Web pages set up by dozens of companies selling kratom leaf and touting it as a way to combat fatigue, pain and depression -- even as an antidote to heroin addiction.

But in addition to its possible medicinal uses, kratom is beginning to show up in U.S. emergency rooms, with doctors saying they are dealing with people sick from taking it — especially teens who try it to get high.

"Every month somebody is trying to get a new 'safe high'," said Frank LoVecchio, medical director of the Banner Good Samaritan Poison and Drug Information Center in Phoenix, Ariz. "(Kratom) is definitely not safe."

Estimating usage of the drug is impossible, but emergency events involving kratom appear to be increasing, he said. In 2005, only two incidents were reported by poison control centers nationwide. But Banner’s center dealt with six emergencies involving kratom in 2011, he said.

As with many herbal and chemical products on the market, science and law enforcement are playing catch-up. Little research has been done to determine the risks of taking kratom, so it remains legal and unregulated in the United States.

The leaf, which is indigenous to Southeast Asia, has been around for thousands of years, and proponents argue that it is safe and effective for many maladies, while having fewer side effects and being less addictive than pharmaceutical alternatives, such as oxycodone. In small doses, they say, kratom provides an energy boost — the plant is in the coffee family — and in larger doses it creates a mellow, sedating effect, acting on the opioid receptors.

"Kratom makes people feel pain free, strong, active and optimistic," according to the Website Kratom.com. It has multiple functions, said the site, which sells kratom leaves, powder and extracts from Thailand — "as a strong and reliable herbal painkiller, to relieve depression and as a social and professional enhancer to intensify communicational skills and induce higher motivation."

Just as its safety has not been well studied, the drug has no scientifically established medical uses — though it has many enthusiastic adherents who swear by it. Testimonials in support of its ability to relieve chronic pain, depression, diabetes and other maladies surface in droves whenever kratom makes the news, as witnessed in the comments following this blog published in the Phoenix New Times in August 2011.

But even promoters warn that daily use of kratom can lead to dependence and nasty side effects.

“Long-term daily high dose kratom consumption is also reported to induce nervousness, sleeplessness, loss of libido, constipation and the darkening of skin complexion,” Kratom.com says in its "dangerous effects" section.

Although there have been no fatalities from kratom, "The known risks and dangers of Kratom overdoses include hallucinations, delusions, listlessness, tremors, aggression, constipation and nausea," the site said.

The emergency room psychiatrist said the patient who recently came in reported using kratom several times a day, every day, "because he discovered that if he stopped it he started getting withdrawal." The doctor said the man's symptoms appeared "identical to heroin withdrawal."

Upon arrival, the patient was suffering "severe depression and anxiety and emerging opiate withdrawal symptoms," including chills, aching muscles and gooseflesh, the psychiatrist said. The patient was treated to ease withdrawal symptoms and then hospitalized, according to the doctor.

Like "bath salts" and "spice" — drugs that are now illegal but were legal and trendy until law enforcers and medical researchers gathered data on their dangers -- kratom is under scrutiny, having been added to a Drug Enforcement Administration's list of "drugs and chemicals of concern."

If the DEA concludes that kratom poses a public health risk, the agency can request that the Department of Health and Human Services place it on a schedule of banned and controlled substances.

The discovery and review process is accelerating in the Internet age, said Barbara Carreno, public information officer for the DEA.

"Things used to get around by word of mouth and it took a long time," said Carreno. "Now anyone can find out about anything within a matter of minutes … so there’s a lot of experimenting with exotic things that no one had ever heard of."

Kratom is illegal in a number of countries in Europe and Asia — most notably Thailand, where much of it is produced. It is now the third most commonly used illegal drug in Thailand, according to the DEA. In that country’s drug culture, the leaf is sometimes combined with cough syrup and Coke, tranquilizers and marijuana to produce a narcotic drink called "4x100."

LoVecchio, of the Phoenix poison control center, said his encounters with kratom are skewed, by definition, because he sees only people who have suffered ill effects, not people who say they are benefiting from it. The ones he treats are typically teens too young to buy alcohol who instead turn to kratom to get high, he said.

"When we see people who take this, they sometimes get respiratory depression," said LoVecchio, similar to the effect of opiates like heroin. "What’s odd is that some of them get really, really agitated, a little combative, (with) nausea and vomiting. They usually get medication for nausea and Valium to ease the paranoia," before being sent home.

He said other users, such as recovered heroin addicts, report that the symptoms are less pronounced, probably because they have built up a tolerance to opiates.

"I would say ban it until a study proves to me that there is a benefit, for anything," LoVecchio said of kratom. "Or restrict it to certain areas, make sure you can control it, make sure people aren’t driving" while using.

For now, kratom is being vigorously marketed in the United States. Some sellers label it as "incense," claiming it is not sold for human consumption while also requiring that buyers be at least 18 years old. One variety being touted for its powerful punch is Maeng Da, which translates to “pimp grade kratom.”

As new vendors get into the market, some of established sellers are trying to encourage self-policing in the industry to avert a ban on kratom.

A Website for the Kratom Association, which claims more than 100 members, has launched a campaign to counter what it describes as harmful and irresponsible representation of the herb — censuring or reporting sellers and head shops that market it as a "legal high," target teenagers or sell kratom adulterated with illegal drugs or other harmful substances.

They are pressing for more research to establish medicinal effects even as they fight efforts to ban kratom.

In one instance, nearly 600 people signed a petition addressed to a Louisiana lawmaker who recently proposed a ban on kratom in his state. Ultimately, Sen. A.G. Crowe of Pearl River withdrew the proposal, but indicated he would call for rules preventing people under 18 from obtaining it.

"Kratom has been used for thousands of years for its medicinal properties. Kratom, when consumed, can treat depression, chronic pain, anxiety, opiate dependence, fatigue, stress and many other ailments," according letter petitioning Crowe. "Besides this it is used by many former addicts of alcohol and opiates. … Comparatively speaking, it is less addictive than coffee."

Austrian saws off own foot to avoid work - report

An unemployed Austrian man sawed his foot off, apparently to avoid being found fit to go back to work.Hours before an appointment on Monday for the labour office to check on his health, the 56-year-old man held his left leg against an electric saw in his home workshop and severed his foot just above the ankle, Austrian broadcaster ORF reported.Bleeding profusely, the man from the province of Styria then threw the foot into an oven, hobbled to his garage and called an ambulance. An emergency operation was unable to reattach the foot, ORF said.

Unruly JetBlue Pilot Locked Out of Cockpit, Flight Diverted

A JetBlue pilot acting erratically was allegedly locked out of the cockpit by his co-pilot, causing an airplane traveling from New York to Las Vegas to be diverted to Texas.

A passenger sitting close to the front of the plane told ABC News that the co-pilot walked out of the cockpit and went to the bathroom before the incident happened.

"The guy was in the bathroom for a while and the pilot inside the cockpit locked the flight door," the passenger said. "Passengers noticed the guy acting weird when he came out of the bathroom."

The unruly pilot then went to the cockpit and started pounding on the door. The passenger heard someone yelling "bomb" and "we're going down." The passenger believes it was the pilot who had been locked out.

A co-pilot remained in the cockpit and was flying the plane, designated as Flight 191, which was carrying 135 passengers and six crew members.

The passenger said it was a "crazy scene" that ended when a group of male passengers restrained the disorderly pilot at the front of the plane until it was able to land.

A government source told ABC News that the pilot was subdued by an off-duty New York City police officer and an off-duty JetBlue pilot who were traveling on the flight.

The off-duty pilot flight helped land the plane in Texas.

"At roughly 10 a.m. CT/11 a.m. ET, the pilot in command elected to divert to Amarillo, Texas, for a medical situation involving the captain," JetBlue said in a statement.

"Another captain, traveling off duty, entered the flight deck prior to landing at Amarillo, and took over the duties of the ill crewmember once on the ground," the statement said. "The aircraft arrived Amarillo at 10:11 am CT, and the crewmember was removed from the aircraft and taken to a local medical facility."

The captain is now in FBI custody.

If 30-minute meals with Jamie or domestic goddessdom with Nigella fail to tempt this evening then it might be worth considering the latest big thing in home cuisine, Cooking with Poo, which has just won the Diagram prize for the oddest book title of the year.

The 114-page cookbook derives its unsanitary title from author Saiyuud Diwong's nickname, Poo, which is Thai for crab. Diwong lives in Bangkok's Klong Toey slum, where she runs a community cookery school. Her book was crowned winner of the Diagram prize following a public vote, beating an array of oddments including Mr Andoh's Pennine Diary: Memoirs of a Japanese Chicken Sexer in 1935 Hebden Bridge; The Great Singapore Penis Panic and the Future of American Mass Hysteria; and Estonian Sock Patterns All Around the World, to win the award.

"Given that this year the three most voted-for works contain the words 'poo', 'sexer' and 'penis' in the title, it appears that this year's prize will go down in history as a blue year," said the award's custodian, Horace Bent. "But there is nothing wrong with that. Many of the world's greatest writers have dabbled in off-colour humour, so I find Cooking with Poo a fitting winner."

There is no prize for the author of the winning title – instead The Bookseller magazine, which runs the award, usually gives a bottle of "fairly passable plonk" to whoever spotted the title. As Cooking with Poo was "unearthed indirectly via Twitter", however, the magazine will be making a donation to Urban Neighbours of Hope, a charity that helped create Diwong's cookery programme.

Diwong may find winning the Diagram award brings other benefits: previous recipient Michael Young, author of Managing a Dental Practice the Genghis Khan Way, said it had the unlikely effect of increasing his literary clout. "Since the award of the prize, I am pleased to say that my writing career is once again flourishing. Editors are keen to publish my words," he said. "Being the winner of the Diagram prize has, I feel, given both the book and me a great deal of credibility, which perhaps without the epithet 'prize-winning' we would never have achieved."

The award has been running since 1978, when it was conceived at the Frankfurt book fair and given to Proceedings of the Second International Workshop on Nude Mice. Previous winners also include Greek Rural Postmen and Their Cancellation Numbers, Highlights in the History of Concrete and Bombproof Your Horse.

NEW YORK (AP) — The new World Trade Center has reached a milestone: The skyscraper being built to replace the terror-wrecked twin towers is now 100 stories high — on its way to becoming New York's tallest building.

Another four feet, and it will surpass the Empire State Building. That should happen within weeks, Steven Coleman of the Port Authority of New York and New Jersey said Monday.

One World Trade Center is expected to be finished by next year, its 104 floors towering over lower Manhattan.

Censors ban 'Shakespeare' film

A new Thai film based on a famous William Shakespeare play has been banned by censors on the grounds that its content may cause disunity among the people.

Shakespeare Tong Tai, or Shakespeare Must Die, is directed by Ing K and Manit Sriwanichpoom.

The film is the first Thai rendition of Macbeth, a bloodstained tragedy in which a Scottish general, with the help of his insidious wife, assassinates a king to pave his way to the throne.

The film includes a contemporary allegory about a fictitious nation where a popular politician rises up the echelons of power.

A document from the Ministry of Culture's Office of Film and Video says that since the film "undermines the unity of people in the country", the censorship committee refuses to give permission to screen it in Thailand.

"The reason given is very broad," Manit said.

"I asked the committee which part of the film fits that verdict and how I should go back to fix it, but they cannot tell me which scene.

"This is a Shakespeare story. It's a tale of greed and lust for power. Since we're banned, I wonder if Thai film-makers are allowed to have opinions, to criticise and to reflect on the reality of the situation.

"There's a lot of talk about democracy, and I don't know how our film is undemocratic."

The film-makers will appeal against the decision.

Shakespeare Must Die runs for 178 minutes and was partly funded by the Ministry of Culture under the 2010 Thai Khem Khaeng stimulus scheme.

It is the second film to have been banned under the Film Act 2009. Thanwarin Sukkapisit's Insects in the Backyard was banned for its nudity and scenes of students prostituting themselves.

The committee that banned the film was chaired by Pol Maj Gen Anek Samplang, who could not be reached for comment. The previous film by Ing K and Manit was a four-hour documentary, Citizen Juling, which looked at the southern unrest and the killing of teacher Juling Pongkanmool. Despite focusing on content that could be deemed sensitive, the film passed the censors and was released in one cinema in 2010.

Bear Saves Robert Biggs From Mountain Lion Attack In California

When Robert Biggs finished gazing at a cute, cuddly family of black bears and turned to continue on his day-hike in northern California on Monday, he thought he was safe.

The 69-year-old man from the town of Paradise was anything but.

He'd been watching a mother bear, her yearling and a newborn from about 40 feet away, but he had no idea that he was being stalked by a ferocious mountain lion. As he turned to leave, the cat pounced on his backpack with all four paws.

"He grabbed me from behind and knocked me to the ground," Biggs told The Huffington Post. "I was on my knees. I had my rock pick out because i was on a steep incline, and I smashed the cat in the head with it. He screamed, but he didn't let go."

His backpack and rock pick were the only things standing between him and certain death, Biggs said. He raised his weapon again for another swing at the hulking feline.

"That's when a blur on my left side grabbed the lion by its throat -- turns out it was the momma bear," he said. "I heard a tremendous screeching, some growling noises."

Biggs said that the bear ripped the cat's grip from his backpack, and the two titans clashed for another 15 seconds. The bear won the battle, probably because it "outweighed the cat 400 pounds to 100 pounds." The cat ran away, and the bear went back down on all fours, humbly making eye contact with Biggs before regrouping with her young.

Biggs -- who had been hiking the same two-mile trail in the Bean Soup Flat area for years -- left with a few scratches and bruises on his arm. Being a mountain man, he refused his wife's pleas that he go to the hospital, and instead put some peroxide on his wounds.

He told The Huffington Post that he owes everything to that bear.

"I'm 100-percent sure it did want to save my life," he said. "We made eye contact. I'd seen the bears before and I know she knew who I was."

Simpsons creator Matt Groening spills Springfield secret

The creator of the iconic cartoon sitcom The Simpsons has finally revealed the inspiration behind the show's fictional town of Springfield.

Matt Groening told Smithsonian magazine he based the town on Springfield, Oregon, but since it is such a common US place name he knew many would think it was their own Springfield.

The Springfield question is one of the best-kept secrets in TV history.

The Simpsons is the longest-running show in the US, on air since 1989.

Characters in the show have often joked about hiding Springfield's real location.

"In anticipation of the success of the show, I thought: 'This will be cool, everyone will think it's their Springfield.' And they do," Mr Groening said.

There are thought to be at least 34 towns or cities called Springfield in the US.

Springfield, WhereverMr Groening also said he was inspired by the TV show Father Knows Best, which was set in a place called Springfield.

But the Springfield in Oregon is just 100 miles (160km) south of Portland, the city where Mr Groening grew up.

The Simpsons has sometimes given false answers to the question of Springfield's true location, leaving open the possibility that Mr Groening's latest comments are a continuation of the joke.

"Whenever people say it's Springfield, Ohio, or Springfield, Massachusetts, or Springfield, wherever, I always go: 'Yup, that's right,'" Mr Groening said.

In one episode, Marge and Homer Simpson's precocious daughter Lisa points to Springfield on a map, but the audience's view is obscured by Bart Simpson's head.

It appears that the town of Springfield, Oregon, has known of its connection to the sitcom since 2007, when Mr Groening visited before the release of The Simpsons Movie.

"Oh, okay, we knew that," a community relations manager told the Associated Press when she heard about Mr Groening's latest comments.

KFC Thailand Apologizes for Facebook Gaffe During Earthquake

While millions of people evacuated the coastline along the Indian Ocean following the 8.6 magnitude earthquake that shook northern Indonesia Wednesday, KFC Thailand insisted that they rush home and order a bucket of chicken.

Local authorities along the Indian Ocean rim braced for a disastrous tsunami, people fled for higher ground. According to the Associated Press, in an inopportune moment KFC posted on its Facebook page: “Let’s hurry home and follow the earthquake news. And don’t forget to order your favorite KFC menu.”

By the time tsunami warnings subsided, hundreds of people began lambasting the company on Thai web pages, prompting the immediate removal of the message. An apology replaced the post, asking for forgiveness for the error.

For many, Wednesday’s earthquakes served as a painful reminder of the devastating outcome of the Asian tsunami in 2004 that claimed 230,000 lives in the same region.

This will fail at some point. It may not be till the European Court of Human Rights, but it will fail. You cannot (within Europe) say that one commercial activity is available to only citizens of that country. There may be more restrictions (as there has already been with higher grade strains and with mushrooms) but they wont restrict to Dutch only.

Australian billionaire to build Titanic replica -CNN

An Australian mining magnate has commissioned a Chinese shipyard to build a replica of the ill-fated Titanic, complete in every detail but equipped with modern technology to prevent a repeat of the original's fateful maiden voyage 100 years ago.

Clive Palmer, a Queensland mining billionaire with strong links to China, told Australian media that he had signed a memorandum of understanding with CSC Jinling Shipyard to build the ship.

He said construction of the luxury cruise ship would begin next year and the ship would be ready to sail in 2016.
